The best way to obtain copies of your records is to call our Medical Record department at 832-355-2077 during business hours, Monday - Friday (8:00 a.m. - 4:00 p.m.). One of our representatives will be happy to assist you.

To obtain a copy of your medical records, you will need to complete this form (). You can deliver the form in person to St. Luke's Medical Records Department located at St. Luke's Episcopal Hospital (in the Texas Medical Center (view map), or you can mail it to the following address:
